    • Acord 125 and 126 forms for GL coverage. Include Acord Forms 131, 140, and 146 also required if Umbrella, Property and Inland Marine are needed. (Also send copies of underlying policies if Umbrella is needed)
    • Supplemental Application – includes detailed descriptions of operations.
    • 5 Years Currently Valued Loss Runs
    • Prior carrier history including effective and expiration dates.
  • Helpful Information
    • Prior policy premium(s)
    • Target premium for upcoming policy
    • Copies of the insured’s current policies
    • Company financials
    • Resumes of company principles
    • Detailed descriptions of any losses
